Hand merge of branch 0.7 with HEAD at tag validator-0_7_4-release

output documentation refreshed, comma tools not in use any more

porting 0.7.4 changes to HEAD:
* bringing back outline, removing parse tree output

listing legacy options, pointing to  semantic extractor for tests beyond outlining

porting all documentation and style (htdocs/...) changes from 0.7.3 to HEAD

* moving all outputs to templates instead of inline code
* adding ucn and soap12 outputs as experimental additions
  ( will allow us to plug unicorn to main validator, and also let people
  actually develop libraries for these outputs)
* deprecating homegrown xml format (in documentation, needs to be in the changelog and announce)

These involve some heavy porting of code from CVS HEAD to 0.7 branch, needs some testing.

Adding a TOC to the user's guide, the error message guide and the help.

This will be validator-0_6_0b1c1 (Beta 1, Candidate 1).

A bunch of fixes to stylesheets and HTML.

Improve navigation for non-graphical UAs (add skip links).

Apply Ville Skytta's mondo relative paths patch. Thanks Ville!

Work around those annoying blank lines left by Apache SSI.

Continure redesign. Add unique titles for each page. Redesign navbar somewhat.
Move options to separate page. Shorten form on front page. Add @title. :-)

Moving to use SSI consistently across the board. Add link to Documentation.

Checking in documentation. Mostly just importing (cleaned up versions of)
Scott Bigham's original FAQ/docs. New additions: a User Manual and information
for developers. All files use the (also new) base stylesheet.
