Actually, I changed my mind. Trying Asir's way presents things at different levels that may not make sense, and my argument at http://lists.w3.org/Archives/Public/www-ws-desc/2004Jul/0287.html still holds.
So I just marked 226 addressed

Did some fixes to address 226: committing them, but about to change this as
per Asir's suggestion. Now that the binding is organized functionally, I agree
with Asir.

Issues 249:
- Prefixed attributes instead of introducing EIIs
- Completely reorganized the HTTP binding to have a functional presentation,
  like the SOAP binding, rather than a purely syntactical [hopefully, I didn't
  break anything moving stuff around]
Issue 166:
- Clarified what gets serialized for a fault it the default binding rules
Removed MIME references: we don't have a binding to MIME
Cleaned up bibliography (what was HTML 4.01 doing there?)
