Regenerate HTML to match revised XSL and DTD

Removed graphic/caption (now using @alt). Removed kw. Added b|u|i as DTD extension. Changed figure number and table format/caption to follow O'Reilly. specref now works for table as well (yes Jean-Jacques, you can now reference a figure ;-). Removed id code in the XSL to reuse the 'anchor' template. Added ids in the spec to prevent them from changing at each publication.

Fixed figures so they don't appear as tables.

Added pseudo-syntax to all but Type and Modularizing sections.

Added the "interface" and "targetResource" attribute on <service>.

Rewrite the service-resource text and merge it with the introduction.

Newer, more complete version of the service/resource diagram. There are now two pictures, an introductory one and a more elaborate picture.

Move the Web Service Definition to a separate section. Will probably need to be spread accross the spec, in individual sections.

The WG decided to provide, for each element, a more digesteable summary of the element's syntax. Here is the first example of  this syntax summary.

Made targetnamespace non-optional in grammar to reflect the prose. Incorporated over comments from Kevin Liu.

Removed prefix for soap11. Removed indication that Part 2 contains binding for SOAP 1.1, since Part 2 currently only mentions SOAP 1.2.
