Improving the "Make book" command: Amaya was formatting the whole document
produced by the Make book command. It now formats only the part that is
visible in the openend views.

For a better mapping with CSS, language P has been extended.  Style and
Weight are now two separate properties.  This allows Amaya to handle
the corresponding CSS properties (font-style and font-weight) independently
from each other and to display text both in italic and bold.

Adding all missing attributes to MathML.

Adding some exceptions to language S: CssBackground, CssClass, CssPseudoClass,
CssId.
