JK: The XPointer XPath expression was broken for embeeded dl lists.

JK: Make annotations work with generic XML documents. N.B.,
we still need to add NS to XPointers to be 100% compliant.

JK: the Xpointer parser crashed when an attribute had a NULL value.

JK: There was a one char shift when making annotations inside the same
line of text.

start point). Small fixes to take into account changes in the selection
for the end points in ranges and when doing a "caret" selection.
